Medication Management in Community Care - Managing Safely

Medication Management in Community Care for Case Managers/Co-ordinators and Direct Care Workers

Medication management is often requested by clients as a part of their suite of services. Providers of home care can assist in supporting clients with medication management. To ensure this service is delivered safely it is essential that it is supported by policy guidelines that include both theoretical and practical competency based training.

This workshop will explore the requirements for offering medication management, including what can or cannot be offered as part of this service.

Key topics covered include:

  • Understand the definition of Medication Management and the difference between medication administration and medication support
  • Be confident in their skills with medication management
  • Establish scope of own ability to provide assistance with medication according to guidelines and jurisdictional regulatory requirements
  • Identify lines of authority to delegate the task, accountability and actions to be taken to handle contingencies
  • Confirm that equipment and documentation are in place for safe medication management
  • Confirm the authority to proceed with assisting clients with Medication Management
  • Follow infection prevention and control procedures

Participants will be given extensive resources to use in the workplace
