ANYONE 40 YEARS OLD AND OLDER IS ELIGIBLE FOR THIS VACCINATION CLINIC. Anyone who qualifies under Phase 1 of the COVID-19 vaccination program is also eligible. Phase 1 qualifying criteria include: Health care workers and personnel who are routinely involved with the care of COVID-19 patients Residents and staff at nursing homes Residents and staff at assisted living facilities Patients and staff at state psychiatric hospitals People with developmental disabilities and those with mental health disorders, including substance abuse disorders, who live in group homes, residential facilities, or centers and staff at those locations Residents and staff at Ohio veterans homes EMS responders Ohioans born with or who have early in live specified conditions that are carried into adulthood, which put them at higher risk for adverse outcomes due to COVID-19* Adults who work in K-12 schools Individuals who are living with type 1 diabetes, pregnant, living with ALS, or bone marrow recipients Childcare service workers Funeral service workers Firefighters Law enforcement and corrections officers *The following are qualifying conditions: cerebral palsy; spina bifida; severe congenital heart disease requiring hospitalization within the past year; severe type 1 diabetes requiring hospitalization within the past year; inherited metabolic disorders including phenylketonuria; severe neurological disorders including epilepsy, hydrocephaly, and microcephaly; severe genetic disorders including Down syndrome, fragile X syndrome, Prader-Willi syndrome, Turner syndrome, and muscular dystrophy; severe lung disease, including asthma requiring hospitalization within the past year, and cystic fibrosis; sickle cell anemia; and alpha and beta thalassemia; solid organ transplants; type 2 diabetes; end-stage renal disease; cancer; chronic kidney disease; chronic obstructive pulmonary disease (COPD); heart disease; and obesity. Online registrations will be reviewed by MCHD staff and appointments will be cancelled if the person who registered is not eligible to receive the vaccine. On the day of the clinic, please bring identification and your insurance card. If you have a Medicare Advantage Plan, please bring your Red, White & Blue Medicare card to the appointment. No out-of-pocket payment will be required. Wear clothing that will allow quick and easy access to your upper arm for vaccination. Your clinic experience will go more smoothly if you complete your registration and screening form before your appointment. The form can be picked up at the office or printed from the MCHD website at https://mchdohio.org/CovidForm. Please do not arrive more than 15 minutes before your appointment time. If you arrive more than 15 minutes early, you will be asked to pull aside or leave the fairgrounds and return at your scheduled appointment time to avoid causing a long wait for those who arrive at the time of their scheduled appointments.

Drive-thru COVID-19 vaccination for people 50 years old or older, who have specific medical conditions, or work in specific occupations.

About this Event
