Advanced Care Planning and Advanced Directives

Advance care planning is not just about old age or end of life care. An accident or an illness at any age can result in physical disability and inability to make important decisions about one's healthcare. Advance care planning is best done when a person is capable to make important decisions about their wishes should they become disabled or terminally ill.

An advance directive is a legal document that goes into effect only if a person is incapacitated and unable to express their wishes. It helps others know what kind of medical care a person would want should they become incapable of speaking for themselves or if they are near the end of life. An advance directive is a dynamic document and can be changed depending on a person's life situation and wishes.
