Laulima: Team-Based Care Learning Collaborative is a multi-island event put on by the Hawai'i State Department of Health for physicians, pharmacists, nurse practitioners, and other healthcare professionals who work with patients with chronic disease. A special focus will be paid to diabetes and hypertension, two of the most common and costly chronic diseases that providers encounter on a daily basis.

The increasing complexity of modern health care has shown the importance of treating and managing chronic disease using a team approach. This event will catalyze interprofessional communication and team-based care by convening providers across a range of practices to demonstrate the expanding role of pharmacists, community health workers, and health care extenders in chronic care management. This event will also build and leverage provider relationships in the shared community; facilitate coordinated interventions by identifying opportunities for care and collaboration; and reintroduce population health as an important lens through which providers operate their practice.

Let's join our many hands for a day of collective action in the realm of chronic disease management.


Light breakfast and lunch are included.

Registration and breakfast begin at 8am.


Who Should Attend:

Educational content is designed to match the current or potential scope of practice of those who work as primary care providers and/or who treat or manage patients with chronic diseases. We welcome physicians, pharmacists, nurses, nurse practitioners, physician assistants, emergency medical service (EMS) professionals and others with a vested interest in providing care to patients with chronic disease using a team-based care approach.

  1. Physicians: Cardiology, Family Medicine, Geriatric Medicine, Internal Medicine, Endocrinology
  2. Nurse Practitioners, Nurses: Cardiology, Family Medicine, Geriatric Medicine, Internal Medicine, Endocrinology, Public Health
  3. Pharmacists: Pharmacology, Medication Therapy Management
  4. EMS Professionals: Emergency Tedical Technicians (EMT), Paramedics
  5. Physician Assistants: Cardiology, Family Medicine, Geriatric Medicine, Internal Medicine, Endocrinology


CME Information:

CME attendance certificates will be distributed after the completion of each Laulima event. Please be sure to complete the following in order to receive your attendance certificate:

  1. Pre-conference survey
  2. Attendance sign-in
  3. Evaluation (distributed at the meeting)


Accreditation Statement:

This activity has been planned and implemented in accordance with the Essential Areas and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of the Hawaii Consortium for Continuing Medical Education (HCCME) and the Hawaii State Department of Health.
