Keeping People Out of the ER

Keeping People Out of the ER will equip human service providers with tools to improve individuals' adherence to medications, appointments, and other doctor's recommendations, such as diet and exercise. Attendees will learn the importance of patient-provider relationships, symptom identification, and communication to prevent ER visits and promote wholistic health and wellness.
Health conditions that may be discussed include but are not limited to:
- Heart disease
- Chronic Obstructive Pulmonary Disease (COPD)
About the Presenter
James Bloomfield, Catholic Charities Community Serivces: Jim is an award winning Health Educator with over 30 years in the Education field. He has 24 years as a certified HIV Educator, 14 years of Case Management experience, and frequently presents on health, safety and quality of life topics in both the US and Canada. He is currently the Project Coordinator for Catholic Charities Community Services.
