Please join Children's Special Health Services for a day focused on the importance of developmental screening and surveillance, care coordination, and health care transition for youth with special health care needs.

Attendance is free.



Healthcare professional workshop topics include:

  • Components of developmental screening and surveillance, and strategies for practice improvement.
  • Service systems and community referrals which support comprehensive care for children and youth with special health care needs.
  • Strategies to improve or implement care coordination services at the practice level.
  • The key elements of youth health transition services and why they are essential to improving health outcomes.

Family Workshop topics include:

  • Navigating transitions for children and youth with special health care needs.
  • Community supports and services for children and youth with special health care needs.

Professional Learning Outcome:

The participant will apply knowledge of developmental screening, early intervention, care coordination and transition support services to provide quality care to the CYSHCN and their families.

Social workers and nurses must be present for the entire 4 hour program and complete an evaluation form to receive CE award.

This continuing nursing education activity was approved by the Louisiana State Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.

This activity has been approved by the Office of Behavioral Health, Louisiana Department of Health, State of Louisiana for 4 clinical contact hours of Social Work Continuing Education. The Office of Behavioral Health is an authorized approval organization of the LA Board of Social Work Examiners.
