For 44 years, Habilitative Systems, Inc. (HSI) has provided quality behavioral health and human services to individuals and families in 15 underserved communities throughout the Chicago metropolitan area. HSI staff respond to the most pressing needs of society, serving persons with mental illness, disabilities, substance use disorders as well as seniors, and children experiencing prolonged trauma.

During the COVID-19 pandemic, HSI responded once again by partnering with the City of Chicago’s Contact Tracing efforts and the Illinois Department of Health vaccination and testing initiatives.

Last year, we joined forces with major health care institutions in Chicago to launch the Westside Health Equity Collaborative which was formed to address critical race and health inequities. We also are an integral partner in the Collaborative Bridges effort to address rising Behavioral Health needs, and suicide in our community.

Please join us at our 2nd Annual “HSI Race and Health Equity Awards” Board Reception on Thursday, December 8, 2022, from 6:00 pm – 8:30 pm. at the DuSable Black History Museum and Education Center, 740 E. 56th Place, Chicago, IL 60637. Your continued support as a corporate sponsor for this event will help us to sustain our vital services, and impact race and health inequities on Chicago’s westside. This year’s event will also support students aspiring to become Community Health Workers, a vital partner in this fight for health equity.

This year’s honorees include the following impactful servant leaders: David Ansell, MD, MPH, Senior Vice President and Associate Provost, Community Health Equity Rush University Medical Center; Kimbra Bell Balark, MD, FACP, Medical Director and Clinical Assistant Professor of Medicine, Northwestern Memorial Hospital; Debra Carey, Principal, Health Management Associates, Inc.; Perri Irmer, President and CEO, DuSable Black History Museum and Education Center; and José R. Sánchez, President and CEO, Humboldt Park Health.

Please contact us with any questions you may have, or you may call Della Akres, Executive Assistant to Donald Dew, at 773-854-8313 to confirm your participation.
