On April 1, 2016 Apple Health Core Connections became the Managed Care Program for youth currently in care and alumni of foster care in Washington State. If a youth was in foster care on their 18th birthday, they are covered through the month of their 26th birthday. The Adolescent to Adult (a2A) overview provides information on how youth can access and use covered physical and behavioral health services. It will provide information on benefits like family planning, transportation to appointments, and over the counter medication. It also gives details about added benefits for our Apple Health Core Connections members such as dollar rewards for going to preventative care appointments, health care coordination, and a no cost cell phone.

Additional topics covered will include:

• What is Medicaid Managed Care
• Why is Coordinated Care the managed care plan for foster/adopt, and alumni (age 18-26)
• We’ll review all covered benefits and how to access (primary care, urgent care, pharmacy, emergency dept, dental, vision, mental health, substance use, etc.)
• What’s the difference between Coordinated Care and ProviderOne
• How to access family planning benefits
• Share info on little known Medicaid benefits, such as you can get condoms at no cost with no prescription at a pharmacy and many over the counter medications are covered with a prescription. Youth/foster parents don’t have to pay for things like ibuprofen, antacids, etc. as long as they have a prescription from their provider.
• Review Coordinated Care’s extra benefits like the CentAccount Rewards ($$ for going to preventative care appointments), Smart Start for Baby, SafeLink Cell Phone (no cost cell phone).
• Talk about how Coordinated Care can help with any barriers to health care services through our Care Management Team.
• How you can help a youth sign up for Medicaid even if they weren’t in foster care on their 18th birthday.
• Community Education team and the trainings we can offer at no cost to the community.
