Guiding the Journey to Success: Best Practices in Case Management

Target Audience

This full-day intensive is intended for caseworkers and program staff within DYCD-funded programs that provide case management services.

Description

Have you ever tried to gert somewhere but forgot the roadmap? Getting from Point A to Point B requires guidance and support, and when working with clients to achieve outcomes, you are often the GPS for clients' journey to reach their final destination.

Effective case management skills allows you to be the GPS guiding your clients as they are in the driver’s seat. Beginning with a foundation in strength-based assessment, this full-day workshop will be highly interactive using exercises, role plays, videos and other practice tools so that you walk away with enhanced skills with which to help clients set meaningful goals and to document their progress.


Learning Objectives:

Participants will be able to:

  • Understand how to conduct a strength-based assessment;
  • Utilize assessment(s) results to help individuals set and achieve important goals documented in their ISP/ISS to improve service delivery and performance outcomes;
  • Gain a better understanding of the value of effective documentation and its impact on individuals and their families;
  • Understand the structure and content of progress notes and have the ability to use the SMARTS and SOAP-D techniques; and
  • Document service interactions stating the essential information about a contact in a manner that adheres to the case management standards.
