Greater Life Apostolic Church In-Person Worship Service Registration

PLEASE NOTE

REVISED REGISTRATION PROCESS: In an effort to continue to remain as safe as possible, we ask that you please register to attend our in-person worship services. Space is limited due to social distancing requirements. All persons ages 2 and up are required to be registered even if you are serving in any capacity. All required questions MUST be answered in order to complete registration.

Please also note that ANYONE WHO ANSWERS 'YES' TO ANY QUESTION PRESENT THROUGHOUT THE REGISTRATION FORM MAY BE DIRECTED TO OUR NURSING/CARE STAFF AND SUBJECT TO ADDITIONAL SCREENING BEFORE BEING ADMITTED INTO THE SANCTUARY.

If you are registered, please make sure to arrive by 10:30am to allow time for the required screening process to take place. If you cannot attend, please be sure to cancel in order to make room for others.

As a reminder, MASKS ARE REQUIRED AT ALL TIMES WHILE INSIDE OF THE CHURCH BUILDING.
