Grand Rounds: DVT/Anticoagulation Therapy

This program will provide participants informatoin on new Core Measures for DVT prophylaxis indications and best practices in regards to Standards of Care for DVT prophylaxis.

PARTICIPANTS SHOULD BE ABLE TO:
* Appropriately follow standards of care for DVT prophylaxis.
* Discuss outpatient therapy and treatment for DVT.
* Implement a multi-disciplinary transition between the inpatient physician adn the outpatient pharmacist to assure continuity of care.
