Join Rev. Melissa Tustin from the Soul Food Project on September 23 and 30th 10-11:30am for two sessions of on-line training in end of life planning and support.

Part 1 will consist of a panel discussion about good end of life care with a DDS nurse, a palliative care physician, an individual with I/DD, a guardianship lawyer/end of life planner and a parent of an individual with I/DD.

Part 2 will focus on the nuts and bolts of creating and following through with a good end of life plan.
