Event purpose: Summarize and provide a framework for the cultural transformation and enhanced capabilities required to achieve improved population health outcomes, as well as highlight the support providers and practice staff need to achieve, not just the Triple Aim, but the Quadruple Aim 

Event objectives:

  • Describe the future of primary care and how it relates to current federal and state innovations

  • Identify ways Michigan’s health care system is transforming in order to deliver population health services

  • Offer practical tips to help clinicians and staff improve their joy of practice, avoid burnout, and embrace cultural transformation

Session Titles:

  • The Future of Primary Care: Transforming Culture to Improve Outcomes, Scott Conard, MD

  • The State Innovation Model (SIM): Testing a More Unified Vision for Primary Care, Phillip Bergquist, MPA

  • Caring for Providers: Building Resilience and Combating Burnout, Elle Ingalls, MBA

Cost: $65 per person ($20 student rate) includes breakfast, lunch, and continuing education credit
