In April, we asked you, the public, and our government colleagues, which forms needed an overhaul. From those 60+ nominations, we picked five to work on at Form-a-Palooza:

• DC Public School Enrollment Packet
• Child Care Subsidy Application
• Certificate of Occupancy
• Food Truck Health Certificate Application
• Disability Parking Space & Placard Application

At the event, you’ll choose one form to work on with a small group of fellow residents. Your goal? To produce a rough prototype of a new and improved version of your form! To help guide you, you’ll hear from the government agency who owns the form, as well as experts in user-centered design, plain language, and behavioral insights.

No experience needed. All are welcome and needed! Free child care is provided for 2-8 year-olds on a first come, first served basis. Please request spot when you register.
