Dear FLPPS Community Navigation Program and PAM Coaching Partners,

Please take this opportunity to review the provided information and register for the training, Using the PAM to Build Trust & Engagement, being held on June 27, 2017 at FLPPS Central, 2100 Brighton Henrietta Town Line Road in Rochester, NY 14623 from 9:00AM to 2:30PM. This workshop is being provided by Bannon Consulting Services. Space is limited to two individuals per agency.

This is a workshop specifically targeted for organization’s participating in the Patient Activation Measure (PAM) project who are Administering the PAM Tool and providing Coaching Services.

* Registration closes at 5:00PM two days prior to the training date.

* Both Breakfast and Lunch will be provided.

Details: Goals for attending this FREE opportunity are:

  1. Understanding the powerful role of CBOs in DSRIP
  2. Understanding the Patient Activation Measure (PAM) connection
  3. Role & benefits of CBOs in implementing the PAM
  4. Develop conversation to identify Community Member needs that are barriers to activation
  5. Identify strategies to improve Community Member activation and engagement

Participants will leave with a Tool Kit including;

  1. Training Slides
  2. Literature Review
  3. Resources Guide Template
  4. Summary of Best Practices
