Who: Healthcare leaders, clinicians, community groups, faith-based organizations, and anyone interested in engaging with thought leaders on dynamic approaches to addressing the social determinants of health.


There are clear associations between social determinants - like a lack of safe and affordable housing, food insecurity, quality education, transportation, economic opportunity, and isolation - and poor health status among children, adults, and seniors. The core goal of this first summit - and of The Root Cause Coalition more broadly - is to more fully engage the healthcare industry in the discussion of social determinants of health, and ultimately, in the development and implementation of data-driven, community-centered solutions to prevent long-term chronic health conditions in the populations that we serve.

Our robust two-day agenda will bring together representatives from healthcare, the faith community, researchers, clinicians, the non-profit sector, and government programs to share best practices, offer community connections and resources, and engage in the crucial discussion of addressing the social determinants of health.

Who should attend this summit?

Anyone interested in learning more about collaborative, data-driven solutions for addressing the long-term health effects caused by various social determinants of health.

What is a social determinant of health?

Social determinants of health have been defined as conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. Social determinants include things like food security, housing, transportation, education, crime, pollution, and discrimination.

Where can I learn more about The Root Cause Coalition, social determinants, and the importance of healthcare and community engagement in collaborative solutions?

What does my registration fee include?

Registration fees include all conference materials, access to conference sessions and breakout discussions, attendance at our December 5th networking reception, and continental breakfast and lunch on December 6th.

What important registration dates should I keep in mind?

Discounted Summit Pre-Registration ($125/person):

  • Closed

Regular Summit Registration ($175/person):

  • Ends: November 13, 2016 (or when event capacity is reached)

Late Registration Availability ($200/person):

  • Beginning November 14, 2016, or sooner if event capacity has been reached, we will open a standby list for those who have not yet purchased a ticket. We will offer late-registration tickets if/when space becomes available.

What is the refund policy?

Should you need to cancel your registration for any reason - the deadline to do so is 6:00 p.m. EST on November 18, 2016. If cancellations are not received by this time, you will be held financially responsible for any and all existing conference fees. Written notifications are accepted via email, fax, or through Eventbrite. Cancellation requests via voicemail are not accepted.

Cancellations made by the deadline of 6:00 p.m. EST on November 18, 2016:

If payment has been submitted, you will receive a refund for the amount paid; however, please note that a $50.00 processing fee will be deducted.

  • Cancellations made after the deadline will not be issued a refund.
  • You may request a conference refund through Eventbrite or by writing to The Root Cause Coalition at mbrigham@rootcausecoalition.org or via fax at (202)266-2629.

  • Email subject line/fax reference line: 2016 Conference Registration Cancellation Request
  • Full name and contact information (phone number and email address)
  • Reason for cancellation (Please indicate if you are a presenter or representative of a Coalition Partner)

What if I need additional accommodations to attend and participate in this summit?

When does the summit begin?

The program begins at 1:00pm on Monday, December 5, 2016 (registration begins at 11:00am).

When does the summit end?

The summit will conclude around 4:30pm on December, 6, 2016.
