Learn how to start and document conversations with family about what matters most to you in a life threatening illness or accident. These conversations lead to shared understanding among family members of your priorities, goals, values, and the designated person (health care agent) to speak for you when you are unable to speak. The Conversation Project and we know This Starter Kit Workshop will not answer every question, but it will help you get your thoughts together, and then have the conversation with your family.
When your health care agent shares what matters most to you in these situations, healthcare providers are able to make treatment decisions that respect what matters most to you. Although optional, and unlike the traditional advance directive and living will documents, we encourage you to submit these documents to the secure mydirectives.org registry. Securing your documents in the mydirectives.org registry will make them accessible to healthcare providers in all 50 states, 44 countries, and cruise ships. No time will be wasted locating this information.
Needed: A willingness to explore what matters most to you in a life threatening illness or accident with facilitators and other workshop participants.
Optional: If you are a note taker, bring a pad and pen.
The event is free, but you do need to register.
Trained Facilitators, Sandra Picot, Seniorcare Nurse Consultant and Revolon Schmidt, Licensed Clinical Professional Therapist will co-lead the workshop.
