Elder Community Collborative- Honoring Choices MA

Health Care Planning Ambassador Workshop


Who Can Attend: This workshop is sponsored by Elder Community Collaborative for their care providers and professionals members, and those been invited by a ECC member. Please do not register if you are not part of the Elder Community Collaborative.

Come join us for a unique workshop and receive tools to:

• Help adults create a Health Care Proxy & Personal Directive (Living Will)
• Educate your group as Ambassadors to engage adults in care planning conversations

Ambassadors are trained in the Honoring Choices MA Getting Started Tool Kit to help adults complete a Heath Care Proxy and Personal Directive (Living Will), and connect adults to needed care. Become the “in-house health care planning expert” and bring this successful program to your organization. The Ambassador Tool Kit provides discussion guides & strategies to engage adults in meaningful conversations at any age and any phase of health, all through their lifetime.

You’ll learn how to-

  • Help adults complete a Health Care Proxy & Personal Directive to start their plan

  • Use the 5 Mass planning documents, including MOLST, to update/add to their plan

  • Start early conversations with adults with memory concern

  • Implement a structured, team-based health care planning model at your organization

  • Offer fun community events with the Ambassador’s “Pizza & Proxy” presentations.

Who can attend:

All Elder Community Collborative Members and those invited by an ECC Member. This workshop is NOT open to Non-ECC members, or those not expressly invited by a ECC member.
