We will be hosting an onsite Immunization Clinic at your location on Friday, October 2nd!

You will need to:

• Choose a time-slot AND SCROLL for immunization options to complete your order
• SIGN UP by Monday, September 28th to ensure the immunizations that you are requesting will be available at the clinic.
• CLICK HERE: Complete the Immunization Consent form - we will also have forms available at the clinic if you are unable to print!
• Bring your Insurance Card to the clinic so that we may bill your insurance for the cost of the immunizations.
