Greetings Ebenezer AME Church Family and Friends,

We are excited to be back in the sanctuary. We will be hosting a limited, in-person Sunday Service on Sunday, May 29, 2022. Registration is required for anyone who plans to attend in-person.

DOORS OPEN AT 6:30AM TO BEGIN REGISTRATION CHECK-IN

To keep everyone safe, Ebenezer will continue to follow COVID-19 Safety Guidelines.

Vaccinations are required. If you are not vaccinated, we encourage you to continue to worship with us virtually. MASKS MUST BE WORN AT ALL TIMES.

COVID-19 SCREENING FORM

By completing this registration, you are acknowledging that you or anyone you have been in contact with have not experienced any of the following in the last 10-12 days:

1. Fever or felt hot?

2. Any shortness of breath or difficulties breathing?

3. Cough, sore throat, congestion or runny nose?

4. Any flu-like symptoms, such as gastrointestinal upset, headache or fatigue?

5. Loss of taste or smell?

6. In contact with any confirmed COVID-19 positive individuals?

7. Been out of the Country in the last 14 days?

**If you answer yes to any of the questions above, please do not attend any in-person worship services.

ASSUMPTION OF THE RISK. By completing this registration, I acknowledge and understand the following: Attending and participating in services at Ebenezer A.M.E. Church includes possible exposure to COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; 2. I knowingly and freely assume all such risks related to COVID-19; 3. I hereby knowingly assume the risk of injury, harm and loss associated with attending and participating in services at Ebenezer A.M.E. Church. I release, indemnify and hold harmless the Church, its Pastors, Officers, Staff, Members, Volunteers and representatives from any claims, demands, actions and costs for any loss, injury, damage or expense whatsoever that might arise out of attending and participating in services at Ebenezer A.M.E. Church.
