Who is eligible to receive the COVID-19 vaccine?

• US citizens, FSM citizens, and green card holders 16 years of age and above are eligible to receive their first dose of the COVID-19 vaccine.

• (16-17 year-olds must be accompanied by a parent or legal guardian to sign a consent form on their behalf)

What vaccines are available?

Pfizer - BioNTech

• Dose 1 for anyone aged 16-17 only
• Dose 2

Moderna

• Dose 1 for anyone aged 18 and over
• Dose 2

The Johnson & Johnson / Janssen vaccine will not be offered at this time.

What do I bring to my appointment?

• If you are receiving your FIRST DOSE of the COVID-19 vaccine, please bring your photo ID, appointment ticket (printed or digital) to your appointment.
• If you are receiving your SECOND DOSE of the COVID-19 vaccine, please bring your photo ID, appointment ticket (printed or digital) and COVID-19 vaccination card to your appointment.
• For 16-17 year-olds, government-issued identification is required to validate age (passport, birth certificate, etc.)

Can I walk-in without making an appointment?

• Walk-ins will be accepted but will be limited to ten (10) per hour. In order to avoid unnecessary waiting, it is best for you to schedule an appointment.

Are there any special considerations in order for me to be eligible to receive the COVID-19 vaccine?

In order to receive the COVID-19 vaccine, you must meet the following pre-screening eligibility criteria:

• Must not currently be infected with COVID-19, or previously exposed within the last 14 days.
• Must not have received any other vaccine, such as the flu shot, in the last 14 days.

How can I cancel my appointment?

Login to your Eventbrite account; click on your profile email on the upper right of the screen; select "tickets". You will see the option to cancel on the left side of the screen (orange button).
