Event description

Description

Health equity is achieved when all people, regardless of who they are or where they live, have the opportunity to live the longest and highest quality of life possible. However, community services like behavioral health, medical care, human services, and public health are complex, fragmented and difficult to navigate. The behavioral health nurse cannot help a depressed mother find affordable housing, the child welfare worker cannot help dad schedule vaccinations, and even a sophisticated case manager (much less a capable but untrained layperson) finds it almost impossible to discern a person’s eligibility for a wide variety of services.

Join us on Oct. 11 for a lecture and discussion with Michael Klinkman, MD, MS, about Social Health Information Exchange (SHIE), a bridge to connect health care delivery and the real-life circumstances in which people live, work and play—the social determinants of health that contribute to our nation’s high health costs.
