A Community Health Worker:

  • Is a trusted member of the community, who understands the ethnicity, language, socio-economic status, and life experiences of their community 

  • Acts as a liaison and provides cultural mediation between health care, social services, and the community  

  • Helps people gain access to needed services

  • Builds individual, community, and system capacity through health knowledge and self-sufficiency

  • Provides services through outreach, patient navigation, community health education and information

  • Provides education, social support, and advocacy to and for the community

The Diabetes Health and Wellness Institute is currently enrolling individuals for a 160-clock hour Community Health Worker Certification Course.

Friday Classes (18 weeks)

Beginning Oct. 14, 2016

8:00 am-4:30 pm

Classroom B

Course will be taught in English

Registration Deadline: Oct. 7, 2016 


$650 class fee due by Oct. 7, 2016
