Description

On January 29, 2013, Governor Dannel P. Malloy, joined by Department of Social Services Commissioner Roderick L. Bremby announced the release of the Strategic Plan to Rebalance Long-Term Services and Supports, a town-by-town projection of Connecticut's long-term care needs and strategies to meet those needs. The plan was largely funded through a combination of federal grants including the Balancing Incentive Payment Program and the Money Follows the Person Demonstration.

On April 20, 2015, the Department submitted an updated strategic plan to the Centers for Medicare and Medicaid Services (CMS) that included a request for funding to continue other key rebalancing initiatives such as improved hospital discharge planning, policy alignment, workforce development, housing, MyPlaceCT, etc. On December 1, 2015, CMS approved the plan and the state received an award for over $129M through 2020.

On December 1, 2017, the Department of Social Services will host a strategic planning meeting to align initiatives within the context of the Department’s larger strategic planning efforts and inform the next 3-year Rebalancing Plan. We have selected you for your content area expertise.
