LABOR RELATIONS ALTERNATIVES, INC.

Since 1984, LRA has provided training and consultation in the development of incident management systems and conducting investigations in 40 states including the District of Columbia. Agencies trained provide care to the ill or disabled, protective services agencies, state and federal agencies with investigatory or oversight responsibilities, and advocacy groups.

Mortality Review and Death InvestigationTraining is a two (2) day training for clinical directors, risk managers, nurse managers, quality assurance staff, investigators and anyone involved in incident management and mortality review processes.

This is an introduction to Mortality Review & Death Investigation that focuses on issues unique to those incidents involving the death of an individual receiving service within the context of an organization's incident management and quality improvement processes. The course will address the following topics:

1. Typologies of death

2. The mortality review process and its relationship to incident management and quality improvement

3. Development of a mortality review policy

4. Medical review

5. Using incident data as part of the mortality review process

6. Explanation: proximate, antecedent and root cause(s) of death

7. Root cause analysis

8. Special problems when conducting death investigations

9. Conducting interviews during death investigations
