About this event
MAXIMUM APPOINTMENTS PER ORDER IS TWO- --EACH PERSON RECEIVING A VACCINE MUST HAVE AN APPOINTMENT.
WHO CAN RECIEVE THE COVID-19 VACCINE?
Governor Ron DeSantis has signed Executive Order 20-315 which outlines that providers can only administer COVID-19 vaccine to the following groups:
- Long-term care facility residents and staff.
- Persons 65 years of age and older.
- Health care personnel with direct patient contact.
- Persons deemed to be extremely vulnerable to COVID-19 by hospital providers.
COVID-19 VACCINE SCREENING QUESTIONS
- Do you have today or have you had at any time in the last 10 days a fever, chills, cough, shortness of breath, difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea, vomiting, or diarrhea?
- Have you tested positive for and/or been diagnosed with COVID-19 infection within the last 10 days?
- Have you had a severe allergic reaction (e.g. needed epinephrine or hospital care) to a previous dose of this vaccine or to any of the ingredients in this vaccine?
- Have you had any other vaccinations in the last 14 days (e.g. influenza vaccine, etc.)?
- Have you have any COVID-19 Antibody therapy within the last 90 days (e.g. Regeneron, Bamlanivimab, COVID Convalescent Plasma, etc.)
If you answered ‘yes’ to any of the above screening questions, please consult with your healthcare provider before making an appointment.
MODERNA COVID-19 VACCINE INFORMATION
The Moderna vaccine has received Emergency Use Authorization from the Food and Drug Administration. The FDA says it is effective at preventing COVID-19 and preventing serious illness if you do catch the virus. Some people may experience symptoms such as fever.

BEFORE YOUR APPOINTMENT
Please bring your ticket number when you arrive for your appointment. If possible, please print and complete the consent form to bring with you to your appointment. Any concerns with your medical history should be discussed with your doctor in advance so you can make the most informed decision for your health. If you have severe allergic reactions that require you to carry an epi pen please bring it to your appointment.

SECOND DOSE APPOINTMENTS
Your second dose of the Moderna COVID-19 vaccine should be approximately 28 days following your first dose. You will be provided with an appointment at the time of your first dose.

If you live outside of Bay County, please consider how you will receive your second dose of the Moderna COVID-19 vaccine. Different regions and States may have access to different vaccine types. Vaccines cannot be mixed. If you start with a Moderna vaccine, you must complete the Moderna series.
