Description

Diabetes and COPD remain top drivers of readmissions throughout the state.

Join us for a train-the-trainer session to learn more about these disease categories and how to prevent readmissions for our patients.

Both programs are provided by Healthcentric Advisors, the New England Quality Innovation Network Quality Improvement Organization.

Upon completion of this session, participants will be able to:

Lung Talk:

  • Engage patients and families in disease self-management
  • Reduce preventable hospital readmissions for COPD population
  • Improve care for our COPD population using 2 major interventions

Managing Diabetes:

  • Identify the six commonly used types of Insulin
  • Describe Insulin Profiles: onset of action; peak action, duration of action, teaching points
  • Use Antidiabetic medication comparison chart to identify % decrease in A1C
  • Identify two diabetes self-management education resources available

Who Should Attend:

Providers interested in learning about collaborative opportunities to improve transitions of care, enhance medication safety and reduce unplanned hospitalizations for patients with Diabetes and/or COPD.
