Morning Session: Connecting the Dots

  • What it is: An opportunity to bring key organizations providing fundamental services together with client navigators, social workers, community health workers, and others to meet and strategize during an interactive event.

  • Why we’re doing it: To help meet the clinical and social needs of the underserved patient population in Bell County by building connections between the people who make referrals and the organizations providing those services.

  • Mid-morning the entire group will be participating in a Case Study exercise. Participants will be grouped together and given 1 of 12 real-lifeclient scenarios. Group members will work together to locate, meet, and strategize with local agencies, that they can refer their client to for help. Vendors will be on hand to share the multitude of services from basic needs to veterans support that are available to help meet these vital needs of our community.

We will hear keynote speakers share best practices on how to incorporate addressing social determinants into a person’s total care of physical, behavioral, and spiritual health to optimize wellness.

Afternoon Session: Social Determinants of Health

  • More information to come- format will be a pannel discussion.
