Description

One of the most persistent problems in healthcare is care transitions between sites of care. A “Gap In Care” is defined as the discrepancy between recommended best practices and the care that is actually provided. Gaps in medical care refer to disparities in healthcare access, healthcare needs and medical services when comparing different populations that fall along racial, ethnic, or socioeconomic lines.

Many patients do not have the clinical education or background to translate the overwhelming amount of information being communicated to them by their health care team. When trying to navigate through such a complicated health care system where communication can be fragmented and misinterpreted, patient outcomes suffer, resulting in unnecessary and excessive health care costs.


Provider awareness and education can help bridge the gap in health care disparities by:

•Helping to improve provider knowledge of cultural and behavioral aspects of health care.

•Helping providers build effective communications strategies.


The main objectives of this training are to provide a detailed overview of the Gaps in Care process, highlight initiatives around Care Transitions, and discuss workflows and processes relative to the HARP Benefit.

Her role at EmblemHealth aims to design, implement, and evaluate quality improvement processes to provide support and drive improvement outcomes for EmblemHealth members.

Caroline Nebocat, LMSW | Clinical Quality Improvement Specialist

Her role at EmblemHealth focuses on improving the quality of care for HARP members through implementation of performance improvement initiatives.
