A full-day conference for professionals, para-professionals and peers who provide care coordination for seniors and adults with disabilities in Medicaid long term care, hospital, primary care, specialty care, mental health, addiction, emergency, outreach and social service settings in the metro area. Our conference theme this year is trauma-informed and culturally-responsive care coordination. There will be several intereactive workshops during our day to build up our skills. Mandy Davis, Dr. James Mason, and O'Nesha Cochran will join us as speakers, along with many esteemed colleagues from different systems leading workshops and conversations.
