3) Profile of a case Manager

4) Job skills and requirement

5) Case Management process

  • post-transitioning communication

6) Identify methods to direct services to meet client needs.

7) Populations served by Case managers

  • People with disabilities

  • People with criminal records

  • People with addictions

  • People with mental illness
