Objectives:
At the end of presentation, the participant will be able to
- Recognize the importance of proper patient follow up & frequent check ins
- Describe proactive approaches to prevention within identified disease populations
- Define the Continuum of care for your patient population
- Identify availability and arrangement of services & resources that will address patient needs
Continuing Education:
Nurses: The Georgia Board of Nursing deems Southwest Georgia Area Health Education Center (SOWEGA-AHEC) as an approved provider for nursing continuing education (CE). This activity is approved for 1.0 contact hour towards the continuing education competency requirement for Georgia nursing licensure renewal. No partial credit offered. Activity #2021-01a.
