Nurses are integral and influential for helping patients navigate within and between healthcare systems. There is a significant need for nurses who can concentrate on care coordination and transition management (CCTM) activities due to the risk of negative outcomes (due to incomplete information exchange, misunderstanding, and missing key factors of strategic plans of care).

Coordinating patient care and managing patient transitions are part of different job responsibilities within nursing (e.g., navigators, care managers, discharge planners, or coordinators).  All nurses have some connection to coordinating care or managing transitions of care for patients.

"I loved the Focused CE Series provided to prep for my CCCTM exam. I passed it on the first try. I thought the modules were very thorough and provided valuable info in order to prepare for the exam, as well as real life operational info to utilize each day in caring for patients. I really enjoyed the webinars by the same instructors who helped prepare the exam. This is a wonderful series and I’m not sure I would’ve passed the exam on the first go around if I hadn’t been so well prepared by" - Beth L., MSN, RN, CCCTM, CHFN, CCP

"This program was engaging and thorough. I passed my CCTM exam on the first try and this was the only study
materials that I used, which is proof that it is a successful program. I am excited to apply what I have learned for the benefit of improved patient outcomes." – Lisa F., MSN, MBA/HCM, RNC‐OB, CCTM

"I have worked in Care Coordination for several years, both on the Acute side and in the Outpatient setting (Home Health/Hospice) and currently with a Accountable Care Organization. I felt that the Care Coordination Certification Prep Focused CE Series was very helpful and was a wonderful resource for studying and reviewing, including the helpful webinars. It was very user friendly!! This CCTM course is a “GOLD Nugget” to help you obtain your GOAL! I would recommend this course to anyone who is planning to work toward the CCCTM!!" - Sherry G., RN, CCCTM

Organizer - Focused CE Series

Focused CE Series combines live webinars with independent web-based study, creating a “virtual classroom” environment.  Tackling complex initiatives, we break them into key topics, and bring nationally known experts right into the attendee’s home, office or facility. Learners go through a multiple week engagement with the same small group of peers, so they can share practices and questions together in an interactive “two-way” webinar experience from wherever they live or work. Between webinars, learners complete independent online study modules to reinforce and apply their new learning and prepare for the next session.
