Join Qualis HealthInsight, the MacColl Center for Health Care Innovation, and the Oregon Rural Practice Research Network (ORPRN), for a free one-day learning event to expand knowledge and skills to improve quality in primary care.

Who: Anyone interested in acquiring practical skills and knowledge about how to develop the capacity of a primary care clinic to improve their quality of care. Suggested roles include: practice coaches or facilitators, quality improvement staff, medical home program managers, and consultants.

Attendees may be employed by a research institution, health plan, medical group, health system, or another aligned organization. While some clinicians participate, the event is intended for those trying to coach and facilitate change.

What: Through the Healthy Hearts Northwest (H2N) project, practice facilitators were able to help over 200 primary care clinics across Washington, Oregon, and Idaho build quality improvement capacity with a focus on cardiovascular disease risk factors, using seven high-leverage changes as a roadmap. Participants will learn about these high-leverage changes as well as gather lessons learned from practice facilitators.

This event will be highly interactive and conducted in a workshop format, using adult-learning techniques to engage participants. You will have the opportunity to share your experiences in quality improvement and practice transformation, as well as learn from others who have shared goals.

By the conclusion of the one-day workshop, participants will be able to:

  1. Identify the seven high-leverage changes needed to build quality improvement capacity and improve clinical quality measures.

  2. Assess the quality improvement capacity of a primary care practice using the Quality Improvement Change Assessment (QICA) tool.

  3. Apply strategies used by experienced practice coaches/facilitators to build QI capacity within a primary care practice.

  4. Connect with individuals from organizations across the region who have similar aspirations and challenges.
