Learning Session One – Cohort Two

Building Care Solutions for Older Adults with Complex Needs (BCS) is a California Quality Collaborative-led, action-oriented learning community targeting the medically complex senior population.

The first of three Learning Sessions for the first BCS cohort is scheduled for September 13, 2017. In a highly interactive workshop learning model, attendees will work closely with experts and peers to develop their existing medically complex programs.

The first Session will cover content areas such as:

  • Critical components of effective medically complex models
  • Identifying the right patients for your program
  • Person-centered care coordination, including patient engagement
  • Provider outreach and engagement
  • Creating a Return on Investment for your program, using an ROI Calculator tool
  • Developing a Team Action Plan

Expert Faculty include:

  • Ann Lindsay, MD, Co-founder, Stanford Coordinated Care Clinic, Stanford University
  • Victor Tabbush, PhD, Health Care Consultant & Emeritus Professor at UCLA Anderson School of Management
  • Barbara Scherrer, RN, BSN, Director, Healthcare Innovations, Santa Rosa Community Health Centers
  • Margie Powers, MSW, MPH, Director, Medically Complex Patients Program, Pacific Business Group on Health
