In February 2016, the Long Island Health Collaborative hosted two Community-Based Organization Summit Events, at Adelphi University and St. Josephs College. During these events, qualitative data was collected about community socio-economics and health needs. This information is being used to support hospital and county Community Health Assessments, prompting plans for community health improvement strategies.
During these events, participants expressed overwhelming desires to continue discussion, network with counterpart organizations, and partner to improve health outcomes. In response, the Long Island Health Collaborative has planned two follow-up events for anyone serving community members on Long Island including: community leaders, faith-based organizations, community-based organizations, care managers, discharge planners, and providers.
