The NC Association of Community-Based ICF/IID and Innovations Providers Association is seeking nominations for the following awards to be given at the 2017 Annual Conference March 28, 29 and 30, 2017:
1. Direct Services Professional Award—This award is given to someone that provides direct services for the individuals we serve and demonstrates the skills and characteristics of a leader, a team player, and above all, an advocate for the individuals we serve.

2. Clinical Support Professional Award—This award is given to the clinical support professional that has consistently set the standard by which all support professionals should perform. This individual will have demonstrated loyalty, teamwork, and leadership, in addition to being an advocate for the individuals we serve.

3. Group Achievement Award—This award is given to a group of individuals who have shown a commitment to assisting, serving, and promoting the rights and needs of those we serve. The group can be a business, club, or other organization that has made a difference in the lives of persons with developmental disabilities.

4. Cornerstone Award for Individual Achievement—This award is given to the individual who has made the most positive impact on people with developmental disabilities. This individual must be an advocate, a leader, and an active voice in the promotion of rights for those we serve. They must have a minimum of five years’ experience in the DD field and be a stable and constant support for those who provide services to the individuals we serve.

General Eligibility Requirements for Nominations:
1. Members of the Executive Committee or Conference Planning Committee are not eligible to receive any nominations or awards.

2. An individual nominated for Direct Service Professional and Clinical Support Professional Awards must be a full-time employee of an Association Member Organization in good standing, or a contractual provider that generates a majority of their income through consultation with Associate Member Organization.

3. No single group home or other related entity of any Association Member Organization, or any nonmember ICF/IID Organization, will be eligible for the Group Achievement Award.

4. An individual nominated for the Cornerstone Award may be an active or retired employee.

To Nominate an Individual and/or Agency, please Submit:
1. The name, phone number, and email of the person making the nomination. 2. The name of the person/organization being nominated.

3. The name of the award category for which the nomination is being submitted.

4. A typed letter (no longer than 2 pages) summarizing why the nominee should be chosen for the award. Up to 2 supporting letters (no longer than 1 page each) from other persons supporting the nominee may also be submitted.

Other:
Each individual/agency may nominate only one nominee per award category.

All nominations must be received by March 20th, 2017
