3. Helo, fy enw i yw Steve ac rydw i'n therapydd Galwedigaethol.

Mae fy arbenigedd yn ymwneud â chefnogi unigolion â nam gwybyddol i gynnal annibyniaeth yn y gymuned. Rwy'n ymwneud ag asesiadau diagnostig o fewn gwasanaeth cof a datrys argyfwng yn dimau iechyd meddwl cymunedol.

Gallaf gynnig cyngor ar strategaethau i gynnal annibyniaeth a lles.

4. Helo, Laura ydw i. Rwy'n Therapydd Lleferydd ac Iaith. Fy swydd i yw darparu cefnogaeth i bobl sy'n byw gyda dementia sy'n cael trafferth cyfathrebu a chysylltu â phobl sy'n bwysig iddyn nhw.

Rwyf hefyd yn cefnogi pobl â dementia sy'n cael problemau gyda bwyta, yfed a llyncu. Rwy'n darparu cefnogaeth a chyngor i bobl dros bob cyfnod o ddementia. Mae gen i ddiddordeb arbennig mewn cefnogi pobl â dementia yn y cyfnodau cynharach i helpu i gynllunio ar gyfer eu dyfodol a mynegi eu dymuniadau, trwy ddulliau arall o gyfathrebu.Mae gen i brofiad personol o ddementia gan fod fy nhaid wedi byw gyda dementia ers blynyddoedd. Rwy'n credu bod hyn wedi fy helpu cael dull mwy teulu-ganolog o ofal dementia.

5. Fy enw i yw Clare ac rwy'n therapydd galwedigaethol. Rwyf wedi gweithio gyda gwasanaethau iechyd meddwl pobl hŷn ers cryn amser! Gweithiwr cymunedol ydw i'n bennaf, felly rwy'n gweld amrywiaeth eang o gleientiaid yn byw gyda dementia ar wahanol gamau. Rwy'n canolbwyntio ar gynnwys yr unigolyn ynghyd â'i deulu/gofalwyr er mwyn nodi'r hyn sy'n bwysig i'r unigolyn hwnnw a gosod nodau.

Gallaf gynnig cyngor ar arferion, dulliau a galwedigaethau ystyrlon sy'n gysylltiedig â gwahanol gamau ar ddementia. Rwy'n gallu cynnig cyngor ar dechnolegau ategol er mwyn cadw rhywun yn ddiogel gartref ac yn trafod strategaethau/offer eraill i sicrhau bod yr unigolyn mor annibynnol â phosib. Mae gen i sylfaen wybodaeth dda o anghenion codi a chario hefyd.

Rwy'n Asesydd Lles Gorau, ac yn hyddysg mewn Trefniadau Diogelu wrth Amddifadu o Ryddid.

6. Helo, fy enw i yw Natalie ac rwy'n Ffisiotherapydd. Rwyf wedi gweithio mewn Gwasanaethau Iechyd Meddwl i Bobl Hŷn ers dros 10 mlynedd ond rwyf wedi cael profiad o weithio ar draws llawer o wahanol feysydd ffisiotherapi. Mae fy arbenigedd yn ymwneud â chefnogi unigolion i gynnal symudiad a symudedd yn ddiogel, cefnogi unigolion gydag ymarfer corff a gweithgareddau sy'n darparu mwynhad ac yn gwella lles meddyliol a chorfforol.

Gallaf gynnig cyngor ar reoli cwympiadau, symudedd, rheolaeth ystumiol, ymarfer corff a lles. Mae gen i wybodaeth dda hefyd am anghenion trafod â llaw hefyd.

7. Fy enw i yw Sarah W. Rwy'n hyfforddwr cymeradwy Cymdeithas Alzheimer ac wedi gweithio dros y pymtheg mlynedd diwethaf gyda thîm hyfforddiant gwaith cymdeithasol ar gyfer awdurdodau lleol Rhondda Cynon Taf a Merthyr Tudful.

Rwy'n bennaf gyfrifol am ddyfeisio cyfleoedd hyfforddi a datblygu ym maes gofal dementia. Rwy'n arbenigo mewn helpu rheolwyr i annog eu timau i newid diwylliant, gan ddefnyddio dull 'Feelings Matter Most' a ddyfeisiwyd gan David Sheard.

Gallaf hefyd eich helpu i 'ddehongli iaith dementia’. Iaith sy'n seiliedig ar emosiwn, nid rhesymeg a rheswm. Gall y technegau hyn leihau straen a phryder yn sylweddol sydd, yn ei dro, yn arwain at well perthnasoedd.

8. Fy enw i yw Heather ac rwy'n Nyrs Iechyd Meddwl Cofrestredig. Rwy'n gweithio ym maes asesu fel y nyrs ymgynghorydd dementia. Rwy’n cefnogi teuluoedd yn dilyn diagnosis i gael gafael at wasanaethau priodol a gyda meddyginiaeth lle bo angen, yn ogystal ag unrhyw faterion teuluol cymhleth sy’n codi.

Mae gen i brofiad personol o ddementia gan fod fy Nhad wedi cael diagnosis o ddementia cynnar pan oedd yn 59 oed. Ar hyn o bryd rwy'n cefnogi fy Mam sydd â sglerosis ymledol (MS) ac sy'n profi dementia fel rhan o ddatblygiad MS.

Rwy'n Therapydd Lleferydd ac Iaith ac rwy'n gweithio gyda phobl sy'n byw gyda dementia sy'n profi anawsterau cyfathrebu a / neu anawsterau llyncu, yn ogystal â'u gofalwyr.Rwy'n siaradwr Cymraeg iaith gyntaf.Mae gen i brofiad personol o berthynas sydd wedi byw gyda dementia fasgwlaidd. Wnaeth hyn rhoi mewnwelediad gwahanol i mi a mwy o ymwybyddiaeth o ddementia ar lefel ymarferol.

10. Helo, Jodine ydw i. Ar hyn o bryd, rwy'n uwch fferyllydd ar gyfer tîm cymunedol lles dementia, sy'n ymwneud â gwasanaethau asesu'r cof. Bues i'n gweithio am flynyddoedd mewn practis meddyg teulu fel presgripsiynydd annibynnol â diddordeb arbennig mewn dementia. Ar ben hynny, mae gennyf gymwysterau therapiwteg cyffredinol a therapiwteg seiciatrig.

11. Helo, fy enw i yw Lexi ac rwy'n ddietegydd sy'n gweithio fel rhan o dîm cymunedol amlddisgyblaethol ar gyfer lles dementia ac rydw i wedi gwneud llawer iawn o waith mewn cartrefi gofal. Rwy'n gallu rhoi cyngor a chymorth ar ddeiet ac archwaeth, gan gynnwys gwneud prydau bwyd yn fwy pleserus i rywun, syniadau am weithgareddau bwyd, helpu gyda sefyllfaoedd gwrthod bwyd neu anawsterau ymarferol sy'n gysylltiedig â bwyta ac yfed neu yn ystod adeg bwyd. Gallaf hefyd gyfeirio at adnoddau ar-lein gyda gwybodaeth a chyngor pellach ar faterion maetheg a dementia.

12. Helo, Jonathan ydw i. Rwy'n weithiwr cymdeithasol gofal lliniarol. Mae hyn yn golygu fy mod i'n gweithio gyda phobl sydd wedi derbyn diagnosis o salwch sy'n cyfyngu bywyd, eu teuluoedd a'u gofalwyr. Rwy'n darparu amrywiaeth eang o gefnogaeth emosiynol, a'r cyfle i drafod pam fod cynllunio ymlaen llaw yn helpu.

Gallaf gynnig cyngor am golli clyw, cymhorthion clyw, tinitws, tactegau cyfathrebu a dyfeisiau eraill a all helpu person i glywed yn well.

1. Hello, my name is Rosey and I am a Speech and Language Therapist working in a Dementia Wellbeing Team.

I am passionate about giving people the best chance to successfully communicate their needs, feelings and wishes in all areas of their lives and providing support for carers in this. I can also help with eating, drinking and swallowing issues.

2. Hi my name is Jan, I am a registered mental health nurse supporting people living with dementia and their families. I am a dementia advisor, supporting people and their families through a diagnosis.

3. Hi, my name is Steve and I am an Occupational therapist.

My expertise is around supporting individuals with cognitive impairment to engage in maintaining independence in the community. I am involved in diagnostic assessments within a memory service and crisis resolution within Community mental health teams.

I can offer advice on strategies to maintain independence and wellbeing.

4. Hello, my name is Laura. I am a Speech and Language Therapist. My job is to provide support for people living with dementia who have difficulties communicating and connecting with people important to them.

I also support people with dementia who have problems with eating, drinking and swallowing. I provide support and advice to people across all stages of dementia, and I am particularly interested in supporting people with dementia in earlier stages to plan for the future and express their wishes, through alternative approaches to communication.

I have personal experience of dementia as my grandad lived with dementia for many years. I think that this helps me to have a more family-centred approach to dementia care.

5. My name is Clare and I am an Occupational Therapist.

My interventions focus on involving the person along with their family/carers to identify what is important to that person and setting goals.

I can offer advice on routines, meaningful occupations and approaches linked with the different stages of dementia. I can offer advice on supportive technologies to keep someone safe at home and look at other strategies/equipment to maximise someone’s independence. I have a good knowledge base of manual handling needs too.

I am a Best Interest Assessor and so have knowledge around Deprivations of Liberty Safeguards.

6. Hello, my name is Natalie and I am a Physiotherapist. I have worked in Mental Health Services for Older People for over 10 years but have had experience of working across many different fields of physiotherapy.

My expertise is around supporting individuals safely maintaining movement and mobility, supporting individuals in exercise and activities that provide enjoyment and enhance mental and physical well-being.

I can offer advice on falls management, mobility, postural management, exercise and wellbeing. I also have a good knowledge around manual handling needs as well.

7. My name is Sarah W. I am an Alzheimer’s Society Approved Trainer, I have lead responsibility for devising training and development opportunities in dementia care.

I can also help you ‘interpret the language of dementia’. This language is based on emotion, not logic and reason. These techniques can significantly reduce stress and anxiety which in turn leads to improved relationships.

8. My name is Heather and I am a Registered Mental Health Nurse. I work in assessment and as the dementia adviser nurse supporting families following diagnosis to access appropriate services and with medication where necessary, and any complex family issues that arose

I have personal experience of dementia as my Dad had young onset dementia and was diagnosed when he was 59. I am currently supporting my Mum who has multiple sclerosis (MS) and is experiencing dementia as part of the progression of her MS.

9. Hi, my name is Helen. I’m a Speech and Language Therapist and I work with people living with dementia who are experiencing communication difficulties and/or swallowing difficulties and their carers.

I’m a first language Welsh speaker.I have personal experience of a relative who has lived with vascular dementia which has given me a different insight and greater awareness of dementia on a practical level.

10. I’m Jodine. I currently working as Advanced Pharmacist for a Dementia Wellbeing community team, involved with memory assessment services. I worked for many years in GP practice as an independent prescriber with special interest in Dementia. I am also qualified in general therapeutics and psychiatric therapeutics.

11. Hi my name is Lexi and I am a Dietitian working as part of a new multi-disciplinary Dementia Wellbeing Community Team and have done a lot of work in care homes. I am able to provide advice and support on appetite and dietary intake, including making mealtimes more enjoyable for a person, food based activity ideas, helping with food refusal or practical difficulties related to eating and drinking or during mealtimes. I can also signpost to online resources with further information and advice in relation to nutrition and Dementia

12. Hello, my name is Jonathan. I am a palliative care social worker. This means that I work with people diagnosed with a life limiting illness, their families and carers. I provide a wide range of emotional support, and the opportunity to discuss why planning ahead helps.

I provide bereavement support, and find that support prior to death can have a positive impact on the loss and grief experienced. I also provide supervision to colleagues and other professionals. I like working with people who struggle with overthinking.

I also have current personal experience of a relative living with Alzheimer’s.

13. Hi my name is Sarah H. I am a social worker, I work predominantly with people living with dementia and their families and carers. I work within a palliative dementia care setting within a community hospice. I can help you ensure a person’s wishes and choices are central to their support now and in the future.

14. Hello, my name is Gemma and I’ve worked as an Audiologist for over 13 years. I work as part of a team who support people who are living with dementia. My role is assess and treat a person’s hearing and communication needs. I can offer advice about hearing loss, hearing aids, tinnitus, communication tactics and other devices that can help a person to hear better.
