2018 Patient Safety Forum –Accelerating Improvement

May 4, Sheraton Framingham

7:30 AM Breakfast and Registration

8 AM – 2: 30 PM Program (including buffet lunch)

Co-provided by the Massachusetts Coalition for the Prevention of Medical Errors and Healthcentric Advisors.

The Patient Safety Forum is focused on accelerating improvement in patient safety, including both organizational strategies that drive improvement broadly, as well as initiatives on specific processes and clinical goals. The Forum will share lessons learned from successful initiatives related to improving effectiveness of clinical teams, reducing ED boarding, improving safety culture through staff engagement, engaging the entire organization in improvement through daily huddles, and more.

Results achieved within initatives on the agenda include:

  • a 25% reduction in ED Boarding Time while admissions and census increased over that same time

  • A 5% increase in safety reports, and 77% decrease in average time to close a safety report, after a six- month implementation of organization-wide safety huddle, as well as many specific safety improvements resulting from the process

  • 18% Increase ( or 7 percentage points) in non-punitive response to errors based on the AHRQ Safety Culture Survey, from a 2 year initiative to build a safety culture through effective engagement of line staff

  • 39% decrease in diagnosis of hospital-acquired C difficile and 47% reduction in C difficile testing, after an new EMR implementation created unintended consequences of high levels of inappropriate testing and overdiagnosis, resulting in unnecessary patient isolation and treatment, wasted labor and materials, as well as exposure to financial penalties

While most of the presenters are reporting initiatives in hospital settings, the organizational strategies and information systems lessons in this program would apply in any healthcare organization working to improve patient safety.

Application for CME credit has been filed with the American Academy of Family Physicians. Determination of credit is pending

The Massachusetts Board of Registration in Medicine approves this training as 5.5 credits Category 1 risk management study.

Nurses: This program meets the requirements of the Massachusetts Board of Registration in Nursing (244 CMR 5.00) for 6.6 contact hours.

Pharmacists: This program has been approved by the Massachusetts Board of Registration in Pharmacy for 5.5 contact hours. It has been given program number 18-051L.
