Have you heard the term social prescribing but really aren’t sure how it works, who it is for and what it involves? This 90 minute webinar will be useful to anyone who wishes to know more about social prescribing, how it works and who is involved.

Social prescribing enables GPs, nurses and other primary care professionals to refer people to a range of local, non-clinical services to support their health and wellbeing.
Recognising that people’s health and wellbeing are determined mostly by a range of social, economic and environmental factors, social prescribing seeks to address people’s needs in a holistic way. It also aims to support individuals to take greater control of their own health.

This session will be useful to anyone who wishes to know more about social prescribing, how it works and who is involved. The target audience includes all people in roles in the clinical workforce who are new to social prescribing, VCSE staff, local authority employees, community volunteers and those working in sectors developing engagement activity including the cultural, green and physical activity sectors. The session will be help you to orientate your own professional practice within social prescribing and to recognise where your organisation may fit within this process.
The session will give a brief overview of the main social prescribing models in England, how they work on the ground and the relationship with the voluntary and community sector. We will outline the role of the link worker and track a typical journey made by an individual from the moment a referral is made to the point they take up an activity.
Potential benefits and individuals will be illustrated with real life case studies and there will be an opportunity to explore the range of activities that are typically involved, including arts activity, volunteering, physical activity, gardening, cookery and social groups.
By the end of the session you will:• Understand how social prescribing works and how it can be accessed by communities• Recognise the potential outcomes for people and communities• Be familiar with the two main delivery models for social prescribing in England • Understand the participant journey • Recognise where your own role and professional practice fits with the social prescribing model.
