NYSHealth Invites You to Attend “A Conversation on Post-Acute Care and the Patient Perspective”

Each year, approximately 1 in 5 hospital patients in the United States, including some 300,000 New Yorkers, require continued care following hospital stays for major surgery or serious illness. Yet too often, patients and their families do not have the critical information and support they need to carefully assess their options and make the best possible decisions.

Please join us for a discussion about the challenges that arise in hospital discharge planning, along with the patient perspective on what can be an exceptionally challenging and difficult process. Findings from a series of NYSHealth-funded reports by the United Hospital Fund on promising approaches for supporting decision-making at discharge will also be highlighted.
