Physicians, nurse providers, pharmacists, physician assistants, specialists, care managers, medical directors, nurses, dietitians, social worker and any one taking care of patients with diabetes.

This activity will provide pharmacists up to 6.25 contact hours. UAN JA4008231-9999-21-041-L01-P has been assigned to this Practice based program for Pharmacists. CE credit information, based on verification of live attendance and completion of the program evaluation, will be provided to NABP within 60 days after the activity completion.

ANCC Designation Statement

Project ECHO® designates this live activity for a maximum of 6.25 ANCC contact hours. Nursing contact hours will be awarded for successful completion of program components based upon documented attendance and completion of evaluation.

DHS Providers: You may use one of your 10 CME days to attend - cancel your clinic or arrange for coverage early.

Upon completion of this session, participants will be able to:

1. Distinguish the key points of the Los Angeles DHS’ Diabetes Expected Practices (practice guidelines)

2. Explain shared decision making for dabetes -prevention

3. Discuss how to engage patients with diabetes to self-manage their disease

This live event at Charles R. Drew University will need to comply with their COVID-19 guideline and require the following in order to gain admittance:

1) Proof of vaccination

2) Negative COVID-19 test (PCR or rapid) within 1 week of the event

3) Temperature check at the event
