In order to receive your CEUs from the 2021 VMAK Conference you need to provide your license information for each state in which you need credits.

Information needed is:

• Your name as it appears on your license;
• Mailing address associated with your license;
• Your License ID or License Number or Certificate Number associated with the state or states you have selected.

Some states may require additional information, such as the last 4 digits of your social security number (SSN). You will be asked for any additional information on the form.
