Description

What: This is a forum for Oregon health system transformation champions to engage in peer-to-peer learning and networking. CCO, clinic and other innovative health system leaders will present projects and discuss learnings focused on three CCO incentive metrics:

  • Increasing effective contraceptive use to reduce unintended pregnancy

  • Reducing tobacco prevalence

  • Reducing emergency department use with a focus on behavioral health

Plenary speakers will include Ignatius Bau, a national expert who provides technical assistance to advance health equity; and Patty Cason, a consultant and clinician trainer who specializes in increasing access to comprehensive patient-centered contraceptive services.

When: May 9, 8:30 a.m.–3:30 p.m. (breakfast and registration 7:30-8:30 a.m.)

Where: Salem Convention Center (Willamette Ballroom C & D - upstairs)

Cost: Free

Lodging: If you require lodging to attend this event, you can reserve a hotel room at The Grand Salem Hotel (attached to the convention center), The Salem La Quinta Inn & Suites (located five blocks from the convention center) or any other hotel in Salem. Please note that we will not have a set of rooms blocked or a discounted room rate available at any Salem hotels for this event.

Call for Projects

Much of the day will be spent in small-group table discussions. If you’d like to present a project that addresses one of the three topic areas, please submit a brief proposal at: surveymonkey.com/r/ICproject

Presentation format: Informal 20-25 minute small-group table discussions. Presenters will share a short project overview, along with a one-page handout, then facilitate a discussion on the topic including project implementation, dissemination and sustainability. Presentations may be repeated.

Important dates:

  • Call for projects open: February 13

  • Project proposals due: March 10

  • Applicants informed of decisions: March 27

  • Presenter preparation webinars (must attend one): March 29 (9-10 a.m.); April 3 (noon-1 p.m.)

  • Project one-page handouts due: April 14

Presentation parameters:

  • Projects must relate to one of the following CCO incentive metrics: increasing effective contraceptive use to reduce unintended pregnancy; reducing tobacco prevalence; or reducing emergency department use (priority given to projects that focus on behavioral health, which could include mental health, substance use, opiate use or other topics).

  • Proposals will be evaluated based on: relevance, innovative ideas, demonstrated outcomes, metrics tied to health equity for key populations, and CCO and geographic representation

  • Presenters must include how projects are financed, challenges and lessons learned

  • Presenters must attend one of two preparation webinars hosted by OHA prior to the event.
