2020 has been a challenging year for all. The COVID19 pandemic has provoked profound and probably long-lasting changes to healthcare systems but also the delivery of educational material to colleagues. Face-to-face conferences have had to be rescheduled and replaced with online meetings, given the continually evolving travel restrictions. The IFAD conference has not been spared these tribulations and we have decided to cancel the physical meeting to replace it by a virtual online meeting.

A neglected topic for way too long, the interest in fluid therapy seems to be quickly rising as the medical community is making a shift from looking at fluids as a mere method of stabilization towards the appreciation of its relevant side effects. However many questions still remain to be answered:

  • Is the upgrade from saline 0.9% to balanced crystalloids worth the extra cost?
  • Does HES still have a place in the OR?
  • Do we have to fill the gap left by HES on ICU with crystalloids, other colloids or even albumin?
  • Is it really impossible to avoid fluid overload by using only crystalloids?
  • Is there still a definitive place for human albumin?
  • How do we treat and monitor specific patient populations, like patients with trauma, liver failure, brain edema and right heart failure among others?
  • How do we avoid a one-size-fits-all regimen in perioperative goal-directed therapy?
  • What with the fluids beyond resuscitation?
  • And what do the authors of the big fluid trials do in real life themselves?

The 1st Virtual IFAD International Fluid Academy Days will again be a 3 day concise meeting on all aspects of fluid managament and hemodynamic monitoring in the critically ill.

Programme

Draft Programme: each day will have 4 sessions with a break in the middle for virtual booth visits. Each session will consists of a live introduction (5min) followed by 2 prerecorded lectures (15 min) followed by a live round table panel discussion. Each session will have additional learning material available in the academy repository. The round table discussions will be interactive with voting and polls. All communication will be done through the Whova platform.

DAY 1 NOV 25

  1. Fluids around the world
  2. Round table discussion: Why are we still talking about fluids?
  3. Maintenance solutions
  4. Round table discussion: Nil-by-mouth from midnight is an archaic concept
  5. Colloids
  6. Round table discussion: Is there really a role for colloids? When do you use colloids?
  7. Closing session Day 1: Fluids in COVID
  8. Round table discussion: What has COVID taught us about fluids?

DAY 2 NOV 26

  1. Monitoring and fluid responsiveness
  2. Round table discussion: The monitors you would have in your unit?
  3. Special circumstances
  4. Round table discussion: Should a general intensivist be able to look after these special circumstances?
  5. Balance, urine and electrolytes
  6. Round table discussion: Are we complicating things, as the body looks after itself?
  7. Closing session day 2: Monitoring in COVID
  8. Round table discussion: Any different from other patients?

DAY 3 NOV 27

  1. Surviving sepsis campaign
  2. Round table discussion: Personalised medicine is just an excuse for clinicians to do whatever they want?
  3. Fluid stewardship
  4. Round table discussion: Why don't you implement fluid stewardship?
  5. Human factors and leadership
  6. Round table discussion: At the end, human factors are more important than technical skills?
  7. Closing session day 3: Organ function and COVID
  8. Round table discussion: What is next in fluid research
