Community Hospice
Community Hospice provides compassionate and quality care, education and support to terminally ill patients and their families, regardless of ability to pay.
Services are provided to patients wherever they call home including private homes, skilled nursing facilities, assisted living communities, or at sixteen-room Alexander Cohen Hospice House.
Community Hospice is Medicare and CHAP accredited and serves Stanislaus, Merced, San Joaquin and Tuolumne Counties.
