Dear Course Participants:
Thank you for your interest in the APTA Clinical Instructor Education & Credentialing Program (CIECP) co-sponsored by The University of Utah, Division of Physical Therapy & The Alaska Physical Therapy Association. Included is a 2-page dossier and payment information.
I ask that you please return both pages of the completed dossier, check or money order for course fees on or before March 23rd 2007.

A late fee of $45.00 is assessed per participant for late registrations.
You will not be registered for the course until both pages of your dossier and course fees are received – APTA members MUST include a copy of your APTA Membership Card and licensed physical therapy providers MUST include a copy of your provider license. The course is open to non-PT providers; except for the credential component.
The course is held on Saturday & Sunday April 14th & 15th 2007 at Providence Sports Medicine & Rehab Therapy, 3801 Lake Otis Blvd #100, Anchorage from 8:30AM - 6:30 PM each date. Participants MUST attend all sessions/times.
