As President of the PCPCC in Washington, D.C., and Global Director of IBM’s Health Care Transformation efforts, Dr. Grundy is the nation’s leading advocate of delivery and payment reform through the Patient-Centered Medical Home model.
Diane Rittenhouse, M.D., M.P.H.
Associate Professor, Department of Family/Community Medicine, University of California, San Francisco
A leading health policy researcher and practicing primary care physician, Dr. Rittenhouse has been at the vanguard of delivery model reform and research efforts. She is a nationally-recognized commentator and author of key works addressing the development and adoption of the PCMH model by practices through the country.

A Louisiana native and practicing physician at the Medical Clinic of North Texas, Dr. Demarie’s practice has gained national recognition for PCMH best practices, innovative care coordination and integration of electronic medical record systems.

Dr. Phenow is a medical executive and board-certified family medicine physician with proven experience in quality improvement initiatives, disease management programs and value-driven alternative delivery models and payment mechanisms.
