SC Coordinated Chronic Disease and Health Promotion Plan Webinar

The South Carolina Department of Health and Environmental Control (DHEC) Bureau of Community Health and Chronic Disease Prevention (BCHCDP) has been awarded the Coordinated Chronic Disease Prevention and Health Promotion Program Cooperative Agreement from the Centers for Disease Control and Prevention (CDC) to, in part, facilitate development of a state Coordinated Chronic Disease and Health Promotion Plan. In close collaboration with DHEC, the South Carolina Institute of Medicine and Public Health (IMPH) is supporting this effort.
On July 12, 2012 from 2:00 PM - 3:30 PM, SC DHEC and IMPH are hosting a webinar to share information about the purpose and development of the South Carolina Coordinated Chronic Disease Prevention and Health Promotion Plan. A survey will be sent out following the webinar to allow for participants and other stakeholders to provide feedback for further state plan development.
We hope that you will take the time to join us for this opportunity to provide input in planning the future direction of chronic disease prevention in South Carolina.
