Attend one of the NYC REACH trainings to learn about the standards, elements and factors you’ll need to meet, to become a PCMH-recognized practice under the 2014 standards. The full-day training includes:

 

  • Best practices on how to transform your practice into an NCQA recognized Patient-Centered Medical Home
  • An in-depth discussion on all the standards, elements and factors that are part of the 2014 NCQA PCMH standards
  • Guidance on succesful preparation of supporting documentation requirements
  • Review of financial incentives tied to PCMH recognition

NYC REACH is the federally designated regional extension center (REC) for NYC. The mission of the REC is to educate and support health care providers in becoming meaningful users of electronic health records (EHRs). NYC REACH is administered by the NYC Department of Health’s Primary Care Information Project (PCIP), a mayoral initiative focused on improving quality care in NYC through health information technology and funded under the Health Information Technology for Economic and Clinical Health (HITECH) Act.

The NYC Regional Electronic Adoption Center for Health (NYCREACH) is a collaboration between the NYC Department of Health and Mental Hygiene and the Fund for Public Health in New York to help make sure doctors in New York City don't get left behind. Our mission is to assist them in adopting technology and methods that measurably improve the health of New Yorkers. Some providers may also be eligible for incentives for EHR use from federal, state, or private programs. Our job is to help offset the transition cost and burden as much as possible through training, education, and links to funding sources.
